Please print and have your ecclesiastical leader (Bishop, Priest, Pastor, Minister, etc...) fill this out. It should be mailed or faxed back to the admissions office.

• Transcripts

If you have fewer than 24 college credits, your high school transcripts are required. With 24 or more college credits, transcripts from all the colleges you have attended are required. Official copies of all your transcripts should be sent directly to the admissions office.

• Test Scores

If you have fewer than 24 college credits, either an ACT or an SAT score are required. If you have taken a test more than once, we require only your highest score. If your ACT or SAT scores are on your high school transcript that is sufficient. Otherwise you need to go to College Board (SAT) or ACT Student (ACT) and request that your scores be sent to us.
